Emergency Readiness Hub
Be proactive, not reactive. What matters most, organized before you need it.
What do you need right now?
Being ready before an emergency happens changes what happens during one.
Don’t miss your follow-up
After a hospital visit or diagnosis, your next appointment is one of the most important steps. If your care team hasn’t already scheduled it, contact your provider today.
Stay on track with medications
Taking your medications as prescribed helps prevent complications and keeps your recovery moving forward. Your pharmacist and care team can help if you need assistance.
Built from real emergency experience
This Hub draws directly on decades of frontline EMS and hospital experience — the same thinking behind the Emergency Life Binder system.
Emergency Readiness Checkup
Takes about 2 minutes. Nothing you enter here is saved or stored.
The public site does not ask you to enter or store your medications, diagnoses, contacts, passwords, financial information, or document contents. This checkup only counts checked boxes in your browser — nothing is sent to SynquUp.

ER, Urgent Care, or Your Doctor?
General guidance to help you decide — not a substitute for calling 911 if you believe this is a life-threatening emergency.
| Situation | Consider |
|---|---|
| Chest pain, trouble breathing, sudden weakness, severe bleeding, loss of consciousness | Call 911 |
| Minor cuts, sprains, mild fever, minor infections | Urgent care |
| Ongoing symptoms, medication questions, routine follow-up | Contact your provider’s office |
| Not sure how serious it is | Contact your provider’s after-hours line, or urgent care can help you decide |
This is general guidance, not a substitute for medical judgment. When in doubt about a serious or life-threatening situation, call 911.
If you end up admitted or discharged from any of these: before you leave, ask for the exact date and time of your follow-up appointment — not just “the office will call you.” Get a number to call to confirm. Being proactive about this is one of the most effective things you can do for your own recovery. Our Recovery Support hub walks through the full 30-day plan.
For Caregivers
Helping someone else stay ready comes with its own questions.
Know where their information is
You don’t need to memorize it — just know where it’s kept, and that you can access it if needed.
Talk about preferences early
Preferred hospital, who to call, and what matters to them — easier to discuss before an emergency.
Keep your own information ready too
If something happens to you, does anyone know how to reach the person you’re caring for?
You don’t have to do this alone
Caregiver support and respite resources are covered in more depth elsewhere on SynquUp.
Trusted Resources
Independently selected — not part of any sponsored placement.
Emergency Life Binder
The organizational system behind this Hub — a patient-held record of emergency contacts, medications, providers, insurance, and documents, all in one place for appointments, emergencies, and hospital transitions.
